Healthcare Hubs are located in Kerang, Quambatook, Pyramid Hill and Boort. They are a collaboration between health services and general practices to provide comprehensive healthcare that better meets the needs of local communities.
Patients will be able to better manage their health while staying closer to home. This is especially important for people with a chronic or complex health condition who may have multiple health professionals involved in providing their healthcare.
As well as a GP and nurse practitioner, a Hub team may include a nurse, diabetes educator, allied health and mental health professionals, and specialists, depending on a patient’s individual healthcare needs.
In addition, patients will also be supported by a care coordinator to ensure their questions and needs are addressed and to help them with making appointments and managing referrals.
The role of nurse practitioners is vitally important in this model of care, as they have the skills, experience, and qualifications to provide holistic health care.
